Thank you for choosing Memorial NeuroSpine for your medical needs.
If you are unable to find a desired time, need to reschedule an appointment, or have insurance & billing questions, please contact the office at 904-296-2522 and we will be happy to assist you.
Please note that all appointments will be confirmed by way of a phone call from our office staff.
All Patients (new and current), please bring the following items:
· A list of current medications · Insurance card(s) and a Photo ID · Your email appointment confirmation · A current referral from your PCP (primary care physician) if your insurance plan requires
· Arrive 20 minutes before your scheduled appointment time with the New Patient Registration Paperwork completed. The New Patient Registration Paperwork can be found in the link below: https://memorialneurospine.com/util/forms/Neurospine-New-Patient-Packet.pdf. If you are unable to print from your computer, please call our office and we will mail the packet to you if your appointment is at least 1 week in the future.
· New patients are required to bring the following:
Imaging films or CD with the reports related to your condition. If you do not have these with you, your appointment will need to be rescheduled. Prior medical records that may be relevant to your visit A list of your current medications Insurance card(s) and Photo ID Completed New Patient Registration Packet
